You can apply for your Commercial Driver License at all Driver License offices except for: Beaver, Farmington, Heber, Kanab, Logan, Provo and Tooele. If the vehicle is over 10,001 LBS you must have DOT Medical Card.
Federal Motor Carrier Administration (FMCSA) has extended the requirement to carry the paper copy of the DOT card until January 30, 2015. Initially drivers only needed to carry the paper copy of the DOT card for 14 days beginning January 30, 2014. All CDL drivers must comply by submitting a DOT card and a Medical Self Certification to the Driver License Division by January 30, 2014 or they must downgrade. Drivers that fail to comply or downgrade will have the driver license disqualified and denied.
