All current CDL holders or any person applying for a commercial driver’s license are required to remit to the Office of Motor Vehicles a self-certification form, a current physical examination, and a current medical examiner’s certificate. In accordance with Louisiana law, ALL CDL drivers, regardless of self-certification category are required to submit a current physical examination form and a current medical examiner’s certificate.
All Louisiana CDL holders are required to self-certify with the Office of Motor Vehicles as the appropriate category. Once the self-certification category has been captured by the Office of Motor Vehicles, it is not necessary for the driver to submit another self-certification unless the driver’s category changes.
